dc.description.abstractСтаттю присвячено питанню досягнення ефективності програмно-цільового бюджетування виходячи з вимог чинного Бюджетного кодексу України. Виокремлено складники ефективності, які, у свою чергу, згруповані за операційною та економічною ефективністю. На прикладі бюджетної програми надання державних медичних гарантій 2020 року за кошти державного бюджету запропонований аналіз за складниками паспорту бюджетної програми аспектів ефективності та сфер відповідальності щодо діяльності відповідального виконавця та ГРК. Зроблено висновки про необхідність додаткового узгодження аспектів ефективності у паспорті бюджетної програми для забезпечення повної доступності медичних послуг. Medical reform of state guarantees for specialized medical services in Ukraine has been postponed. The paper considered insufficient availability of the medical services under the program of medical guarantees for the second level of medicine as one of the reasons. Efficiency problem with financing of these services in program-target budgeting appeared. The authors purpose was to identify and analyze the components of the effectiveness of the program-targeted method of managing budget funds on the example of medical guarantees, ensuring the availability and quality of medical services in accordance with public policy objectives. Research methods are empirical observation, analysis and synthesis of the budget program components content, the theoretical method of convergence from the abstract to the specific characteristics of the medical guarantees effectiveness in general; the proposals were made using the method of specification. The main results of the research were to identify the components of the efficiency of the programtarget method in budgeting, grouping them by organizational efficacy and economic effectiveness aspects. It was assumed that the second-level medical reform did not take into account the full accessibility of medical services as part of medical guarantees. This assumption was confirmed by analyzing the content of medical guarantees program for financial support components; subsequent synthesis of the budget program’s each components effectiveness aspects was processed. The conclusion is about a weak operational efficiency as well as a combination of performance indicators of the opposite in terms efficiency of budget savings for each case and efficiency in terms of the services quality in the budget program with an increase in the number of medical guarantees. This creates the ultimate effectiveness of the program as a guarantee of more cases. However, the task of the program is to provide affordable, high-quality and effective medical services; it is impossible to achieve it with the current content of the program and the responsible executor’s powers. We offer further scientific research to ensure the effectiveness of medical services through competitive activities, but in terms of guaranteed support of the medical infrastructure system. Статья посвящена вопросу достижения эффективности программно-целевого бюджетирования исходя из требований действующего Бюджетного кодекса Украины. Выделены составляющие эффективности, которые, в свою очередь, сгруппированы по операционной и экономической эффективности. На примере бюджетной программы предоставления государственных медицинских гарантий в 2020 году за средства государственного бюджета предложен анализ аспектов эффективности и сфер ответственности по деятельности ответственного исполнителя и ГРК по составляющим паспорта бюджетной программы. Сделаны выводы о необходимости дополнительного согласования аспектов эффективности в паспорте бюджетной программы для обеспечения полной доступности медицинских услуг.uk_UA
